Pediatric and Adolescent BH Clinician

General Description:

The Pediatric and Adolescent BH clinician is responsible for delivery of integrated behavioral health, individual counseling, and case management services to the Pediatrics and Behavioral Health Department. This position includes providing case management, follow up, and coordination between primary care providers, other Caring Health Center (CHC) departments, and external sources such as schools, psychiatric hospitals, and other internal and external collateral contacts involved with families and children. The Pediatric and Adolescent BH Clinician will provide Behavioral Health services which include BH Intake screenings, triage for walk-in patients, and treatment within the primary care medical home.

Supervises: N/A

Salary/Status: non-exempt

Minimum Requirements:

1. LMHC, LMFT or LADAC, or Master’s level, licensed eligible in the state of Massachusetts

2. CANS licensed and experience working with clients under the age of 18

3. Ability to work independently as well as to collaborate with Integrated Care Team.

4. Familiarity with multicultural groups, previous work experience in a multicultural setting.

5. Prior experience addressing the BH needs of patients with chronic medical conditions (e.g., diabetes,

hypertension).

6. Experience with quality improvement and tracking/reporting on BH indicators.

7. Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse

audiences.

8. Ability to demonstrate professionalism and respect; regard for patient privacy is essential.

9. Organizational skills a must.

10. Ability to work independently and with a team.

11. Computer literacy.

12. Experience with an Electronic Health Record is preferable.

13. Bi-lingual strongly preferred but not required.

14. Experience with facilitating group therapy is a plus.

Essential Responsibilities and Duties:

1. Provide therapeutic interventions including adult and child individual, group, and families.

2. Provide Behavioral Health Screening Brief Intervention Referral and Treatment services to CHC patients.

3. Coordinate and collaborate with the pediatric primary care team (including providers, pharmacist, medical assistants, etc.) to improve health outcomes.

4. Provide “warm hand off” for identified patients when schedule allows.

5. Maintain professional, accurate and timely clinical documentation of patient encounters, and services provided to each patient as well as track BH indicators as identified in the BHI grant.

6. Attend BH meetings weekly as well as other identified meetings for patient care.

7. Attend supervision with BH Director as scheduled.

8. Other duties as assigned to support the mission of the organization.

9. Develop and build upon existing collaborative relationship with external agencies related

to child and family social service’s needs.

Working Conditions:

  • Position requires prolonged periods of sitting at a desk and typing on a computer; ability to stand for prolonged periods of time; ability to perform moderate activities such as climbing stairs, bending, stooping, reaching, and lifting up to 20 pounds at times; and potential exposure to adverse outdoor environmental conditions. 
  • CHC has multiple offices and this position may be required to report to one or more locations; infrequent travel may be required. 
  • We are an active healthcare facility which provides an array of services to the community; this position may, at times, come in contact with sick patients. 
  • May require extended hours to support department initiatives.
